SUBJECT: Request for Proposal - Student Accident Insurance
Culpeper County Public Schools is requesting proposals to provide individual voluntary Student Accident Insurance to students in grades K-12 and to provide an all Athletic Insurance Plan for the senior high schools. The latter plan should be for all students involved in the school athletic program grades 6-12. Approximately 900 students participate in the following sports: baseball, basketball (girls/boys), cheerleading, cross country (girls/boys), field hockey, football, golf, outdoor track (girls/boys), soccer, softball, tennis (girls/boys), wrestling, lacrosse, debate, forensics, and theater festival. Some students participate in several varsity activities but are not counted more than once.
Culpeper County School Board will award the Student Accident Insurance K-12 and the athletic insurance to one company or to two separate companies depending on which is most advantageous to the school system. Other pertinent information concerning Culpeper County Public Schools is as follows:

Proposals are to be received or brought to the School Board Office, 450 Radio Lane, Culpeper, Virginia 22701, by 2 p.m., Thursday, May 14, 2009. Proposals should be marked "Proposal - Student Accident Insurance (Attention: Pearl Jamison)". Culpeper County Public Schools reserves the right to award this bid for a period of up to three years if the bid remains constant.
